Before filling out a registration form, check you are in our catchment area.
You can also find out which GPs you can register with on the NHS website.
Registering with the surgery
To register with the surgery:
- use the NHS Register with a GP: Haverfield Surgery service
- If you are unable to do this, you can visit the practice to complete a paper form. Please bring both photographic ID and proof of address with you.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Temporary patients
Patients living in England
If you are temporarily visiting and require GP services, your regular GP practice is often the best option, as they have access to your medical records and current medications. They can arrange a telephone appointment with an appropriate clinician and can send your prescription to any pharmacy of your choice.
Consider calling NHS 111. They can assess your situation and, if necessary, direct you to an Urgent Care Centre or A&E, which may be better suited to your needs.
If 111 or your regular GP practice are unable to help you, we may be able to register you as a temporary patient. These registrations are for on the day acute medical issues only.
Patients living outside of the UK
Patients visiting from overseas my be treated under the NHS, please seek advice from NHS 111
Named GP
Your named accountable GP is Dr Ciobanu. Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not always be available and if your needs are urgent it may be necessary to discuss them with an alternative doctor.
